To complete the SSA-3369 form, you must apply for Social Security Disability Benefits. The application procedure for disability benefits is time-consuming and complicated. The legal paperwork may be confusing for the applicants.

  • You enter your hours in the timesheet instead of scheduling and payroll.

  1. Maintain consistency with your answer: The reviewers will search for consistency in your answers. Example: In one answer, you are saying you are unable to stand for long, and in another, you are saying you are able to cook your meals, which are both contradictory.

How do I fill out the Social Security Work History Report (Form SSA-3369)?

Section 1: Personal information

Start filling out the form by providing all the basic information, such as:

Section 2: Previous work

You need to elaborate on how your disability is affecting your capability to work or perform work at your job. Instead of stating that every day was the worst, you could describe your duties and explain how some days were the worst, others were average, and some were good. You need to outline all your regular responsibilities, including your work in the reception area, along with the tasks that are part of the job. You can also include any cleaning or security tasks in this section.

If you are unable to perform these duties, please include this information in the form. It will help determine that your medical condition is severe and is preventing you from doing any job for the SSA.

There is a checkbox for answering such types of questions. The word frequent refers to how much weight you can easily and regularly lift in your job.

Managing people is one of the transferable capabilities. The SSA may deny your application if they think you have the management skills. They may suggest that you look for such work instead of applying for a disability claim.

Section 3: Remarks

This is the final section of the form. This section includes a large space for remarks. You are free to use this section to answer any of the previous questions were long and could not fit in the provided space.

If someone else is filling out your work history report, there is an option for them to mention their name, address, and date at the bottom of the form.
